Anal fissure: Ten-year experience

ABSTRACT
Objective:To assess the experience in the medical-surgical management of anal fissure.
Setting:Private praxis.
Patients and methods:We reviewed the clinical records of 226 patients with a diagnosis of anal fissure from January 1994 to May 2004. Patients were divided in two groups. Group 1, 130 patients with acute anal fissure. Group 2, 96 patients with chronic anal fissure. We assessed: location of the fissure, symptomatology, type of treatment and complications.
Statistical analysis:Central tendency measures and chi-square.
Results:Group 1, 124 patients (95%) received first line medical treatment, and in 6 patients (5%) second line medical treatment was started. Group 2, 91 patients (95%) received surgical treatment and 5 patients (5%) were started on second line medical treatment. In group 1, 8 patients did not respond to the first line medical treatment; in group 2, 1 patient abandoned the second line treatment due to secondary reactions. From the surgically treated patients, only 3.8% presented complications that were resolved favorably and no recurrences occurred.
Conclusion:The acute anal fissure responds adequately to medical treatment in its different modalities within a period of 8 days. The chronic anal fissure can be treated with medications that decrease the hypertonia of the anal sphincter, but sphincterotomy is superior to any treatment.
