Experience in the management of bile duct cysts

ABSTRACT
Objective:To identify the frequency and discuss the surgical treatment of hepatic ducts cyst.
Setting:Third level health care hospital.
Design:Descriptive transversal study.
Patients and methods:We reviewed all the clinical records of patients with a diagnosis of cysts of the hepatic ducts from 1985 to March 2004. We recorded, age, gender, age at presentation of symptoms, clinical symptoms, diagnostic methodology, surgery performed, morbidity and mortality.
Results:We studied 33 patients with diagnoses of hepatic ducts cyst. Of them, 25 (75.7%) were women and 8 (24.3) were men. Age at clinical presentation was: before the age of 20 in 8 (24%), from 20 to 30 years in 14 (42%), more than 30 years in 11 (33%). Anatomically, the cysts were classified according to Alonzo-Lej Modified by Todani: type I, 23 (70%); type II, 6 (18%); type IV, 4 (12%), there were no type III and V cases. Surgically, resection of the cyst was performed, reconstructing with a Roux-en-Y hepato-jejuno-anastomosis in 27 (82%) patients. Morbidity was of 12% and mortality of 6%. Average follow-up was of 58.3 months. In-hospital stay was of 9 days.
Conclusion:The bile ducts cyst is not common in our milieu. In this study, it predominated in women with an average age of presentation of 32 years. the choledochus variety mostly found corresponded to type I.
