Colonoscopy in the diagnosis of low digestive hemorrhage.Ten-year experience

ABSTRACT

Objective: To show the experience of a center specialized in colon and rectum treatment with the endoscopic study of hemorrhage of the low digestive tract.
Setting: Specialties Hospital “Siglo XXI”.
Design: Retrospective, descriptive, transversal study.
Statistical analysis: Central tendency and dispersion measures. Chi-square to establish differences in proportions.
Material and methods: All colonoscopy studies performed in 10 years were analyzed. The analyzed variables were: age, gender, background endoscopic diagnosis and complications, analyzed in a general way, and dividing the group in younger and older than 60 years.
Results: One-thousand-forty-six colonoscopies were performed in patients with hemorrhage of the low digestive tract; 48.27% corresponded to men and 51.7% to women with a mean age of 63.5 years (16-96). The main etiology was diverticular disease in 32.56%, and 37.28% corresponded to normal colonoscopies. One intestinal perforation occurred. Proportion analysis revealed that women have a 2.76 times higher risk of bleeding due to intestinal inflammatory disease than men (p ‹ 0.018). Men older than 60 years have a 6.35-times higher risk of bleeding due to diverticular disease (p ‹ 0.01). Patients with infectious colitis younger than 60 years present a 2.60 times higher risk of bleeding (p ‹ 0.001) and those older than 60 years with ischemic colitis have a 4.24 times higher risk of bleeding than those younger than 60 years (p ‹ 0.01). Conclusions: Diverticular disease is the main cause of hemorrhage of the low digestive tract at the hospital where the study was conducted (Specialties Hospital “Siglo XXI”). Incidence of complications was of 0.09% and the diagnosis was established in 63% of the patients.
