The use of advanced bipediculated flap to reconstruct nasal and upper lid defects, after the excision of basal cell carcinomas. A modified technique and other uses

ABSTRACT

Background. The bipediculated flap from the glabelar skin described by Field is an excelent method to close nasal bridge defects.
Objective. To use this tissue in the same area from the original description and to apply it in other parts such as in areas away from the nasal tip and also in the treatment of upper lid tumors.
Methods. An original technique is described with a personal modification, pointing out its advantages and disadvantages.
Results: We prefer the design of two identical triangles, same size, that support their base in each side of the circle previously designed and draw with a disposition that allows us to make an spindle afterwards and the corresponding suture. The technique is illustrated in the cases localized to the nasal dorsum, far away from the tip and in the upper lid. We recommend it to avoid the flap use in the involved nasal area or to dissecated too much tissue.
Conclusions. The bipediculated flap is an ambulatory surgery, with slight morbidity and its technique is simple, the final scar is insignificant and the cosmetic results are excellent. The complications can be infection, hypertrophic scar, suture dehiscence, necrosis and tissue retraction.
