Cystic with a right hepatic duct drainage. Case report

ABSTRACT
Introduction: The anatomy of the bile ducts is complex and is characterized by the frequent presence of anatomical variations of the intra or extrahepatic structures, this can be found in 50% or more of the general population. The abnormalities on right bile ducts implantation are the most frequent and one of the main risk factors for surgical injuries during cholecystectomy. Abnormal drainage of the cystic duct in the right posterior hepatic duct or common hepatic duct can be found in 0.58 to 5% of patients.
Case report: 31-year-old female patient, with postprandial colic type pain, located in right hypochondrium. Ultrasound with lithiasis in the gallbladder, without dilation of the intra or extrahepatic biliary tract. She was scheduled for an elective laparoscopic cholecystectomy and it was decided in the transoperative to perform trans-gallbladder cholangiography due to an anatomical variant at the cystic duct drainage, finding an abnormal bile duct anatomy with the cystic duct draining in the posterior right hepatic duct and this, directly at the common bile duct (Blumgart F).
Conclusions: The anatomical variants of the biliary tract are frequent. The transoperative cholangiography is a resource that should be used more frequently for a proper identification of the bile duct in order to avoid biliary tract lesions.
