Experience with revascularization of the dorsal artery of the foot in diabetic patients, in a second level health care hospital

ABSTRACT
Objective: To assess the efficacy of revascularization by means of distal grafting of the dorsal artery to salvage the ischemic diabetic foot.
Setting: Second level health care hospital.
Design: Prospective, longitudinal, observational study without control group.
Statistics: Percentages as summary measures for qualitative variables.
Material and method: Revascularization of the dorsal foot artery was performed in patients with demonstrated neuro-ischemic lesion of the foot and/or pain at rest. The direct variable to be assessed was rescue of the foot, besides we evaluated age, gender, associated diseases, smoking, donating vessel, type of applied graft, permeability of the graft according to type, post-operative follow-up time, complications, and mortality.
Results: Forty distal grafts were performed at the pedal artery in 37 patients with neuro-ischemic lesions, 39 patients with pain at rest and 1 with changes in color. Primary permeability was of 57%, the average salvaged feet was of 77% and mortality was of 5 patients. Survival was of 88% with an average follow-up time of 18 months.
Conclusion: Direct revascularization of the pedal artery in diabetic patients is an effective and feasible procedure, and can be performed in hospital considered as second level health care centers in our country.
