RESUMEN

La dermatosis pustulosa erosiva de la piel cabelluda (DPEPC) es una enfermedad inflamatoria crónica e infrecuente que afecta principalmente a adultos mayores, que resulta en una alopecia de tipo cicatricial; es desencadenada por agresiones locales como trauma, intervenciones médicas y medicamentos tópicos o sistémicos. Se manifiesta con pústulas, erosiones, úlceras superficiales y costras en la piel cabelluda. Las principales opciones de tratamiento incluyen corticosteroides tópicos, inhibidores de calcineurina, terapia fotodinámica, entre otros, aunque con recurrencias frecuentes al suspenderlos. Presentamos el caso de una mujer de 71 años con una úlcera de larga evolución localizada en la piel cabelluda, sometida a múltiples tratamientos y con una evolución recidivante, en quien, tras un abordaje integral, se integró el diagnóstico de dermatosis pustulosa erosiva de la piel cabelluda.
