ABSTRACT
Erosive pustular dermatosis of the scalp (EPDS) is a rare,
chronic inflammatory disease that primarily affects older
adults and can result in cicatricial alopecia. It is often triggered
by local insults such as trauma, medical interventions,
and topical or systemic medications. It presents
with pustules, erosions, superficial ulcers, and crusting on
the scalp. The main treatment options include topical corticosteroids,
topical calcineurin inhibitors, photodynamic
therapy, among others, although relapses are common
after discontinuation. We present the case of a 71-year-old
woman with a long-standing scalp ulcer who underwent
multiple treatments with a relapsing course, in whom an
integral diagnostic workup ultimately led to the diagnosis of erosive pustular dermatosis of the scalp.
