Uso de Serenoa repens en el tratamiento de la caída del cabello: Una revisión narrativa

RESUMEN

La alopecia androgenética y otros tipos de pérdida de cabello representan un problema altamente prevalente con importante impacto psicosocial, cuyo manejo se ve limitado por la eficacia parcial y los efectos adversos de terapias aprobadas como minoxidil o finasterida. Este trabajo ofrece una revisión narrativa de la evidencia preclínica y clínica de Serenoa repens (saw palmetto), para la pérdida de cabello. Se sintetizan datos que muestran efectos antiandrogénicos (inhibición de la 5α-reductasa, modulación de la señalización androgénica), así como acciones antioxidantes, antiinflamatorias y su influencia sobre los factores de crecimiento implicados en la biología del folículo piloso. Se revisan estudios intervencionales y observacionales que evalúan formulaciones orales y tópicas de Serenoa repens en alopecia androgenética y efluvio telógeno, los cuales sugieren estabilización de la enfermedad y mejoras en densidad, calidad y caída del cabello, con un perfil de seguridad generalmente favorable. No obstante, la heterogeneidad de las formulaciones, dosis y diseños de estudio limita la comparabilidad y la validez externa de los hallazgos. Se discuten las motivaciones de los pacientes hacia terapias “naturales”, las implicaciones regulatorias de su condición de suplemento y se identifican prioridades de investigación futura, incluyendo ensayos controlados y plataformas avanzadas de liberación tópica.
