Use of Serenoa repens in the treatment of hair loss: a narrative review

ABSTRACT

Androgenetic alopecia and other forms of hair loss are highly prevalent conditions with a significant psychosocial impact, whose management is limited by the partial efficacy and adverse effects associated with approved therapies such as minoxidil and finasteride. This article presents a narrative review of the preclinical and clinical evidence on Serenoa repens (saw palmetto) for hair loss. Available data indicate antiandrogenic effects through inhibition of 5α-reductase and modulation of androgen signaling, as well as antioxidant and anti-inflammatory properties and an influence on growth factors involved in hair follicle biology. Interventional and observational studies assessing oral and topical formulations of Serenoa repens in androgenetic alopecia and telogen effluvium suggest disease stabilization and improvements in hair density, hair quality, and shedding, with a generally favorable safety profile. Nevertheless, heterogeneity in formulations, dosing regimens, and study designs limits comparability across studies and restricts the external validity of current findings. Patient motivations toward “natural” therapies, along with the regulatory implications of Serenoa repens being classified as a dietary supplement, are discussed. Finally, priorities for future research are identified, including well-designed randomized controlled trials and the development of advanced topical delivery platforms.
