Lesión meniscal en asa de cubo crónica y aislada: ¿meniscectomía o reparación meniscal?

RESUMEN

Introducción: las roturas meniscales en asa de cubo representan 10% de las lesiones meniscales. Aunque tanto la meniscectomía como la reparación meniscal son tratamientos válidos, persiste la controversia sobre cuál es la mejor opción en casos crónicos. Este estudio tiene como objetivo describir y comparar los resultados clínico-funcionales y la tasa de complicaciones o reintervenciones en pacientes con roturas crónicas y aisladas en asa de cubo tratados mediante meniscectomía o reparación meniscal. Material y métodos: se realizó un estudio en pacientes intervenidos entre 2012 y 2021 en el Hospital Universitario Infanta Leonor. Se incluyeron pacientes con lesiones crónicas y aisladas en asa de cubo, tratados mediante meniscectomía o sutura meniscal. Se analizaron variables demográficas, clínicas, tiempo hasta la cirugía, seguimiento postoperatorio y resultados funcionales mediante las escalas IKDC y Lysholm-Tegner. También se registraron complicaciones y reintervenciones. Resultados: se incluyeron 30 pacientes, 23 tratados con meniscectomía y siete con reparación meniscal. El grupo de meniscectomía mostró una mejora del IKDC de 39.1% preoperatorio a 88.8% al año y 86.3% al final del seguimiento (cuatro años), sin complicaciones ni reintervenciones. El grupo de reparación mostró mejoras más discretas: IKDC de 38.9 a 82.3% en cinco años, pero con una tasa de fracaso de 42.85% que requirió meniscectomía parcial. Conclusión: ambos tratamientos son válidos, pero el retraso en la reparación meniscal puede aumentar el riesgo de complicaciones y reintervenciones. Es necesario mejorar los protocolos de actuación ante estas lesiones.
