Cervical facet subluxation, return to basics, the key to diagnosis

A 68-year-old male patient with chronic cervical pain and limitation to cervical extension and an antalgic attitude in hyperflexion, for which cervical spine X-rays were requested, with sclerosis and facet subluxation at C3-C4 and C5-C6 as key findings in the diagnosis of this pathology. A systematic approach involving the definition of the vertebral lines and the vertebral bodies, elements of the posterior arch, and intervertebral spaces helps to detect these anomalies, among other findings that may distract the observer.
