Surgical approach and follow-up of giant liver cysts in older adults: case report and literature review

ABSTRACT

Hepatic cysts are rare lesions. Most are asymptomatic and do not require treatment. However, when they grow large, they usually produce symptoms and require definitive resolution. Among the various surgical techniques described, laparoscopic unroofing is currently considered the technique of choice. We present the case of a female patient with a giant hepatic cyst measuring approximately 6,000 mL, presenting with postprandial pain and fullness. Due to the high risk of rupture, we placed a pigtail catheter for controlled outpatient drainage and subsequent laparoscopic unroofing and marsupialization; the procedure was successful, with no reported complications.
