Discopexy in the treatment of anterior disc displacement of the temporomandibular joint: clinical case report

ABSTRACT

Introduction: internal disorders of the temporomandibular joint are conditions in which the articular disc shifts from its original position, causing pain or dysfunction. When conservative treatment is ineffective, surgical treatment such as arthroscopy is the ideal choice. Objective: to present an alternative surgical treatment for these disorders when the necessary equipment for arthroscopy is unavailable, as well as to disseminate the open surgery technique for disc replacement, which has been used successfully. Case presentation: a male in his second decade of life with a medical history of Ehlers-Danlos syndrome, which was diagnosed using magnetic resonance imaging as anterior disc displacement with reduction of the right mandibular condyle and anterior disc displacement without reduction of the left mandibular condyle. He was previously treated with conservative management for seven months without clinical improvement. Because our hospital does not have an arthroscope, open surgery was performed for disc replacement and eminectomy. Two 7-mm self-drilling titanium 2.0 miniscrews were placed posterior to the lateral pole of the condyle, secured to the articular disc with 2-0 Prolene suture. The outcome was successful, as our patient's painful symptoms were resolved and the temporomandibular joint function was restored. Conclusion: our technique proved useful in the absence of the necessary equipment for arthroscopy.
