Multidrug-resistant Pott´s disease in a two-year-old pediatric: case report

ABSTRACT

This clinical case describes a two-year-old pediatric patient diagnosed with multidrug-resistant Pott’s disease. The patient initially presented with nonspecific symptoms that progressed to difficulty walking and the development of kyphotic deformity in the thoracic spine. Spinal imaging studies revealed a pathological fracture of the T12 and L1 vertebrae and the presence of a paravertebral abscess with a subdural appearance, in contact with neural elements. An abscess aspiration was performed, and polymerase chain reaction (PCR) testing along with an acid-fast bacilli (AFB) culture were requested. Both tests confirmed the presence of multidrug-resistant Mycobacterium tuberculosis. The patient was initially treated with anti-tuberculosis medications; however, due to a poor clinical response, surgical intervention was performed. In the short term, the patient showed favorable progress, with pain relief and correction of the kyphotic deformity, ultimately improving the patient’s quality of life.
