Mucinous neoplasia of the appendix: review and update on the approach

ABSTRACT

Introduction: appendiceal mucocele is a rare pathological entity characterized by cystic dilatation of the vermiform appendix due to the accumulation of acellular mucus. Its incidence is low, reported in only 0.2 to 0.7% of appendectomies. Given its rarity and the risk of a serious complication –such as pseudomyxoma peritonei–, proper diagnosis and management are crucial. Case report: we present the case of a 35-year-old man who was incidentally diagnosed with appendiceal mucocele. The patient was being evaluated for an unrelated condition, cholelithiasis, and the mucocele was discovered during the diagnostic evaluation for this condition. Despite this finding, the mucocele was asymptomatic. Results: surgical management consisted of laparoscopic cholecystectomy to treat cholelithiasis, performed simultaneously with an open appendectomy to remove the appendiceal mucocele. Conclusions: appendiceal mucocele is a rare finding that requires a high level of suspicion, especially when diagnosed incidentally. Management should be surgical, prioritizing complete removal of the appendix without rupture to minimize the risk of pseudomyxoma peritonei. The presence of other abdominal pathology, such as cholelithiasis in this case, highlights the importance of a comprehensive patient evaluation.
