ABSTRACT
Central giant cell granuloma (CGCG) is a rare benign neoplasm, most commonly observed in females and typically located in the mandible. Its peak incidence occurs between the second and third decades of life; however, it may also present in older individuals, although reports in patients over 65 years of age remain scarce. This article describes the case of a 78-year-old totally edentulous patient with multiple systemic comorbidities, diagnosed with CGCG. The difficulty in determining the aggressiveness of the lesion, the lack of response to pharmacological management, and the patient's functional rehabilitation needs led to the planning of surgical treatment. A comprehensive assessment of the patient's systemic health and sociodemographic context is essential for selecting the most appropriate therapeutic approach in such lesions. Future studies should reconsider the criteria used to assess the aggressiveness of CGCG in aging populations.
