Anesthetic management of a newborn with congenital lobar emphysema undergoing lobectomy: a case report

ABSTRACT
Introduction: congenital lobar emphysema (CLE) is a rare pulmonary malformation that may cause severe respiratory failure in the neonatal period. Anesthetic management is challenging due to dynamic hyperinflation and cardiovascular collapse risk.
Case presentation: a 14-day-old female newborn with left upper lobe CLE (ASA III E) underwent thoracotomy and lobectomy with inhalational induction, protective ventilation, and permissive hypercapnia.
Results: intraoperative hemodynamic stability was maintained. The patient was transferred to Neonatal Intensive Care Unit (NICU) and successfully extubated at 48 hours postoperatively.
Conclusion: early lobectomy with a protective anesthetic strategy offers excellent prognosis. Early pleural opening and permissive hypercapnia are cornerstones of management.
