Electromyography patterns and clinical correlation in carpal tunnel syndrome

ABSTRACT

Introduction: carpal tunnel syndrome (CTS) is the most common mononeuropathy, affecting the median nerve. It can impair the complex movements of the wrist and hand through alterations in sensitivity and strength, affecting productivity and quality of life. The use of electromyography, or electrodiagnostic testing (EMG), in conjunction with clinical evaluation, is important for determining nerve damage and for excluding other conditions. Material and methods: this was an observational, descriptive, cross-sectional, single-center, and retrospective study. It included 50 patients of any age, with diagnosis of CTS and underwent EMG without any history suggesting secondary nerve damage. Information was collected including motor and sensory nerve conduction studies, as well as needle electromyography, for the median and ulnar nerves. The correlation between the clinical findings and/or symptoms reported and the EMG results was studied. Results:a clinical correlation was found between the presence of a positive Phalen test and the myographic analysis, showing a statistically significant association (p ‹ 0.05). This highlights the importance of performing this test during the examination of patients with suspected CTS. However, no correlation could be determined between the nerve conduction studies or needle electromyography findings and the Tinel test or the symptomatic characteristics reported by the patients. Conclusions: using EMG for the study of CTS is essential to determine the characteristics of the nerve injury. It is important to note that utilizing various diagnostic methods helps achieve an accurate diagnosis, avoid false positives, generate a prognosis, and/or provide timely management.
