Quantitative hepatic elastography in evaluation of normal subjects and subjects with non-alcoholic fatty degeneration. Inter-observer correlation

ABSTRACT

Introduction. Elastography is an image method which characterizes the rigidity or deformation of soft tissues in a given structure. It is a new method for assessment of hepatic fibrosis, thyroid gland nodules, mammary nodules, and hepatic and renal tumors.
Material and methods. A transverse, prospective, prolective, and observational study, conducted in patients with and without changes due to fatty infiltration, assessed with qualitative ultrasound and with quantitative elastography with Acoustic Radiation Force Impulse (ARFI). The study was conducted in 40 patients at Hospital General de Mexico divided in five study groups. Laboratory samples were taken to record blood sugar at baseline and at 120 minutes, Matsuda index, triglycerides, total cholesterol, HDL cholesterol, and LDL cholesterol. The goal was to detect inter-observer variability and correlation with sonographic studies.
Results. Metabolic parameters were contrasted in patients with quantitative and qualitative steatosis by Student t and statistically significant difference (p ‹ 0.05) was found in the parameters: BMI, Matsuda index, triglycerides, and HDL cholesterol. An intra-class coefficient of variation was made to analyze the level of concurrence between the two observers, the result of which was 0.82 (p ‹ 0.001). Taking the normal limits of elastography, quantitative assessment was made of non-alcoholic steatohepatitis in dichotomic form (absent or present) with Spearman’s Rho correlation, obtaining R = 0.63 (p ‹ 0.001). A binary logistic regression was made between elastographic values (m/sec) and qualitative steatosis with result 3.48 (p= 0.015)
Conclusions. There was an acceptable correlation between measurement with qualitative ultrasound and ARFI elastography in patients with non-alcoholic steatohepatitis; Spearman’s Rho of 0.63 (p ‹ 0.001). There was adequate intra-class correlation of observers: 0.82 (p ‹ 0.001).
