Biliary tree compression secondary to hepatic cyst

ABSTRACT
Introduction: Most hepatic cystic lesions have a benign course and only a small number of cases unveil with a relevant clinical morbidity or mortality. Biliary tree compression has been described as one of these clinical scenario. Case report: twenty seven years old woman that starts in December 2010 with dull abdominal pain located in the upper right quadrant, nausea with no vomiting or jaundice as main complaints. The physical examination shows hepatomegaly 2 cm below the costal margin painful with deep palpation. Direct hyperbilirubinemia and abdominal ultrasound that showed a complex septated hepatic cyst within segment V. Computed tomography showed a hepatic mass suggestive of a hydatid cyst with compression and displacement of the extrahepatic biliary tree. Laparoscopically an 11 cm hepatic cyst was found in segments V and VI and a wide deroofing was performed with no biliary tree involvement. Conclusions: Biliary tree compression due to a hepatic cyst must be considered a rare clinical entity. Laparoscopic approach of this hepatic lesions provides safety, quickness and excellent results due to lack of the biliary tree manipulation.REFERENCES
